The Pennsylvania Department of Health regulations require the exclusion from Pennsylvania schools of any students who do not meet the following minimum immunization requirements:

a. Four doses of Diphtheria and Tetanus (one dose administered on or after the fourth birthday).

b. Three doses of Polio vaccine.

c. Two doses of Measles, Mumps and Rubella (MMR) vaccine, one after 15 months of age and a second dose of Measles vaccine (preferably given as MMR).

d. Three doses of Hepatitis B vaccine, the first two doses given one month apart and the third dose six months after the first dose. **(currently Grades K,1 & 7)

e. Evidence of Varicella (Chicken pox) immunity:

1. Date of Varicella disease.

2. Varicella vaccine per the Pennsylvania Department of Health- one dose of Varicella vaccine administered at 12 months of age through 12 years of age; or two doses of Varicella vaccine administered at age 13 years or after.

**All new students are also required to show proof of a tuberculin skin test on or after January 1st of the year of entrance to school. If a student received BCG as an infant, or within the last 18 months, a mantoux skin test must be administered and the results recorded in mm of induration. If greater than 10mm induration, a chest x-ray is required, and INH therapy is recommended. If INH treatment was received, please report the dates.
